How do I get help from Life Works?

Patience and persistance will be required to obtain approval for State payment of your services. 

Don't give up.  Call for help when needed.

State Paid services are authorized by the Washington State Department of Social and Health Services (DSHS). 

To become eligible for the State to pay for our services, you must become a client of the Division of Developmental Disabilities.   Click here for a link to the eligibility requirements and a description of the process.

To Apply for State eligibility please call 1-800-339-8227 and request an eligibility packet.  You will need to follow the instructions with the packet to complete it and mail it in. If you need assistance filling out the packet, please ask for it at this number.

Once you become elegible for DDD services, you will be assigned a Case Manager through the local DDD office at 711 Vine Street in Kelso, WA.  The Case Manager will discuss uptions for services and the process to get approval for a referral to Life Works.  Feel free to call the Life Works office at (360) 577-9093 if you have questions along the way.

Training for family or community members is available on a variety of topics including:

  • The Basics of Developmental Disabilties
  • Seizures
  • Positive Behavior Supports
